House Democratic Leader Hakeem Jeffries has delivered the line again and again, most recently in testimony circulated this week: when it comes to Medicaid and undocumented immigrants, he says, the federal taxpayer is not spending anything at all.
"Under current law, undocumented immigrants are already ineligible for Medicaid, the ACA exchanges and SNAP benefits," Jeffries said. "Not a single undocumented immigrant in this country gets a dime in federal taxpayer dollars for any part of comprehensive Medicaid coverage. Not one."
The federal government's own books tell a different story. Under federal law — 42 U.S.C. 1396b(v) — a program called Emergency Medicaid pays hospitals, with federal dollars, for emergency treatment of people who would qualify for Medicaid except for their immigration status, including emergency labor and delivery. In fiscal year 2023 alone, that spending came to roughly $3.8 billion. It is a small share of total Medicaid spending — under 1 percent — but it is federal money, spent through Medicaid, on patients in the country illegally. That is considerably more than "not a dime."
And it adds up. A House Budget Committee analysis backed by Congressional Budget Office figures put emergency services for undocumented migrants at $16.2 billion in federal and state spending from 2021 through mid-2024. A 2024 federal audit separately found that California had improperly claimed $52.7 million in federal Medicaid funds for undocumented immigrants — money Washington was never supposed to pay at all.
To be precise about the record: Jeffries leans hard on one word — "comprehensive." Routine, full Medicaid enrollment is indeed barred to undocumented immigrants under federal law, and states like California that chose to offer them full coverage did so with state-only funds; Gov. Gavin Newsom has boasted that California is "one of sixteen states to provide care to people regardless of immigration status." But the federal Emergency Medicaid channel is real, it runs through the very program Jeffries was describing, and it costs taxpayers billions.
Washington has already acted as if the money exists. The 2025 reconciliation law reduced the federal matching rate for Emergency Medicaid starting October 1, 2026 — a provision that makes no sense if no federal dollars were flowing. Jeffries, who is in line to become Speaker if Democrats win the House, leads a party whose agenda includes reversing the law's Medicaid cuts.
So the argument is not really about whether federal Medicaid money reaches undocumented immigrants. The government's audits, its spending records, and its own new law all say it does — $3.8 billion in a single year through the emergency door alone. The only question left is why the man who would run the House keeps telling taxpayers the figure is zero.
Never miss a story.
